Chronic Migraines, Headaches and Occipital Neuralgia

What Are These Conditions?

Chronic migraines, tension headaches, and occipital neuralgia are debilitating nerve-related conditions that can persist for months or even years.

These may manifest as:

  • Pain in the back, side, or front of the head
  • Scalp sensitivity
  • Migraines triggered by light, smell, or noise
  • Nausea, dizziness, and loss of quality of life
  • Long-standing symptoms despite medical treatment

What Triggers These Conditions?

  • Surgical trauma (e.g., craniotomy)
  • Physical trauma (car accidents, hits, falls)
  • Anatomical compressions of nerves
  • Long-term nerve irritation or entrapment

Understanding Chronic Migraines, Headaches and Occipital Neuralgia

Common (Often Ineffective) Initial Treatments

Patients may present with:
  • Pain medications (e.g., NSAIDs, triptans)
  • Physical therapy
  • Supportive observation
However, these approaches frequently fail because they do not treat the underlying nerve damage or entrapment.

Why Early Referral Matters

"Clinical outcomes progressively worsen the longer the delay between injury and surgery."

  • Severity of nerve injury is directly tied to how well a patient can recover
  • Early evaluation and intervention improves long-term outcomes

(Mackay BJ et al., Annals of Plastic Surgery, 2021)

Patient Experience – Survey Results

A large group of chronic nerve pain sufferers reported:

%

wished they had been referred sooner

%

were never informed that nerve surgery was an option

%

were told that nothing could be done

%

were told nerves would recover, but it would take a year or more

Barriers to Care

  • Insurance companies often classify nerve surgery as experimental, despite clear clinical evidence
  • This leads to denial of coverage, prolonged suffering, and reduced quality of life

Impact on Opioid Use and Medication Dependence

Studies show a significant decrease in opioid and medication use after surgical intervention:

  • Felder & Ducic, PRS Global Open, 2021–2022
  • Nerve surgery improves function and reduces drug dependence in chronic cases

When to Consider Surgical Evaluation

If you have suffered from chronic migraines, headaches, or occipital neuralgia lasting more than 3 to 6 months despite specialized medical care, a surgical nerve evaluation may be warranted.

Still in pain after surgery or trauma?

It might be nerve-related. Schedule a consultation and get an expert evaluation.
