Thoracic Outlet Syndrome

About Thoracic Outlet Syndrome (TOS)

Primary causes:

  • Traumatic injuries
  • Anatomical abnormalities causing nerve compression
  • Surgical complications

Common symptoms:

  • Pain in shoulders, arms, hands or fingers
  • Numbness and tingling sensations
  • Muscle weakness in upper extremities
  • Neck pain
  • Potential migraine headaches

Understanding Thoracic Outlet Syndrome

Diagnostic considerations:

Requires exclusion of:

  • Brain/spine pathologies
  • Other systemic conditions causing brachial plexus compression

Standard treatment limitations:

Conservative approaches (medication, physical therapy) often fail to address nerve compression.

Surgical Management

Key publication:
Ducic I, Felder JM, Quadri HS. Common Nerve Decompressions of the Upper Extremity: Reliable Exposure Using Shorter Incisions. Annals of Plastic Surgery, 68:606-609, 2012.

Surgical approach:

  • Focused decompression of affected nerves
  • Targeted release in lower neck/supraclavicular region
  • Utilization of shorter incisions for reduced morbidity

Critical Treatment Timing

Evidence-based findings:

  • Outcomes decline with delayed intervention
  • Recovery potential inversely correlates with:
    • Injury severity
    • Time since symptom onset

Patient Care Challenges

Reported issues:

%

of patients felt referral delays occurred

%

weren't informed about surgical options

%

received incorrect prognosis about spontaneous recovery

%

were told no treatments existed

Insurance barriers

Despite peer-reviewed evidence, many insurers classify nerve decompression as experimental.

Surgical Evaluation Criteria

Consider consultation if experiencing:

  • Confirmed TOS diagnosis
  • Persistent upper extremity symptoms
  • Duration exceeding 3 months
  • Failed conservative management

Still in pain after surgery or trauma?

It might be nerve-related. Schedule a consultation and get an expert evaluation.
